 Hi Thomas ,

   I dont have an assembled form for you but I have a whole bunch of
useful elements that you could borrow to make your own. I've got *loads*
of straight lines ( which are particularly handy since you can rotate
them around and they change from columns in to rows magically).I've only
got black ones at the moment , but I could order some coloured or dotted
ones for you - sometimes it takes a few weeks to get the order back
though, since I think they make most lines in Taiwan nowadays.So if time
is an issue I would stick to black.
  I also have some boxes (sqaure or oblong) that are really handy , and
most numbers and letters. If you can do without using any 'a's or 'q's
and steer clear of number 6 then youre pretty well covered.
  You may need to hire some protective clothing due to the rather toxic
adhesive that you'll need - alternatively , you could get a freind to do
all that bit while you supervise from a safe distance. Ordinary
household windows can be a real blessing here.
   The only snag is that I cant seem to find the manuals or tutorials
,so you may have to learn as you go. There are a few bugs listed in the
release notes that are worth looking out for , a particularly nasty one
can happen if you're photocopying a form and youre facing the same
direction as the page when you hit the copy button. This sometimes
results in hitherto perfectly straight lines becoming curved or rotating
randomly.

 On another note , the 'request for cpu time' thingy you mentioned can
be circumnavigated by using this simple script .(irix only)
